Amazon Warehouse Worker Death Sparks OSHA Investigation in PA

OSHA is leading an investigation into the death of Jody Rhoads, who died while working at the Amazon Fulfillment Center in Carlisle, PA.  

Rhoads, 52, was pronounced dead at the Carlisle Regional Medical Center.  The Cumberland County Coroner said that a motorized pallet jack Ms. Rhoads was operating crashed into some shelving.

In a statement, Amazon spokeswoman Kelly Cheeseman said, “Our thoughts and prayers go out to Jody’s family and loved ones.  We are actively working with OSHA to investigate this tragedy.”

Kevin Kilp, area director for OSHA, said an investigation began Wednesday.  The investigation could take as long as six months:

“We respond to a report anytime there is a workplace fatality,” Kilp said.

During an investigation, OSHA officials determine whether the employer violated any OSHA safety standards, resulting in the death. If they did, that may lead to the issuance of citations, Kilp said.

“We take pictures, video, interview employees and management, and generally collect information pertinent to the investigation,” Kilp said.
